Complementary and Alternative Health Care
Client Bill of Rights (for participants)

This document is provided to you in accordance with a state law that requires that persons receiving services from complementary and alternative health care practitioners be informed of their rights.

1. Minneapolis Pathways, Inc. (Pathways) is located at 3115 Hennepin Avenue South, Minneapolis, MN 55408. Phone: 612-822-9061. pathwaysminneapolis.org. 
   
   The practitioner providing services to you is a Volunteer Provider at Pathways.  Any questions about your rights should be directed to a Pathways’ staff member.
   
   
2. When scheduling each session, you can find information about Pathways’ Volunteer Providers’ degrees, training, experience, and other qualifications on the session request page within the online scheduling system.
   
   The State of Minnesota has not adopted any educational and training standards for unlicensed complementary and alternative health care practitioners.  This statement of credentials is for informational purposes only.
   
   Under Minnesota law, an unlicensed complementary and alternative health care practitioner may not provide a medical diagnosis or recommend discontinuance of medically prescribed treatments.  If a client desires a diagnosis from a licensed physician, chiropractor, or acupuncture practitioner, or services from a physician, chiropractor, nurse, osteopath, physical therapist, dietitian, athletic trainer, or any other type of health care provider, the client may seek such services, which are not provided at Pathways, at any time.
   
   
3. Some Volunteer Providers are interns and work under the direction of an on-site supervisor. If your provider is an intern and you have any concerns about the services that you receive, the on-site supervisor will be able to address them.  If your concern arises after you have left the Pathways’ building, please contact a Pathways’ staff member.
   
   
4. Pathways’ participants have a right to file a complaint regarding any Volunteer Provider by writing to or speaking with Pathways’ Staff. Any complaint regarding the Pathways’ staff may be reported by writing to the President of the Pathways Board of Directors.  Please address correspondence to: President of the Pathways Board of Directors, 3115 Hennepin Ave. S., Minneapolis, MN 55408.
   
   
5. Pathways’ participants also have a right to file a complaint with Minnesota Department of Health Office, Health Occupations Program, Office of Complementary and Alternative Health Care Practitioners, PO Box 64882, St. Paul, MN 55164-0882. Phone: 651-201-4200 Email: HOP@state.mn.us.
   
   
6. The majority of Pathways services are offered free of charge.  In the event that a program or service includes a fee, the amount of the fee, payment terms and opportunities for refund will be clearly stated in Pathways Calendar or other literature.
   
   
7. Pathways’ participants have a right to reasonable notice of change in services. If a Volunteer Provider cancels a one-on-one service for reasons other than illness, an attempt will be made to reschedule the service in a timely manner.
   
   
8. Pathways does not offer medical diagnosis, mental health diagnosis, medical treatment or mental health treatment. Our educational services are aimed at empowering the individual to discover within themselves ways to support their healing journey, physically, emotionally, mentally, and spiritually. Your Volunteer Provider will provide you with information about the services that you will be receiving.  Any questions you have about the services that you will receive should be directed to your Volunteer Provider, including any questions about recommended services and the duration of such services.
   
   
9. Pathways’ participants have a right to expect courteous treatment and to be free from verbal, physical, or sexual abuse by Pathways’ staff and volunteers.
   
   
10. Pathways does not tolerate discrimination based on race, color, creed, religion, national origin, sex, marital status, disability, sexual orientation, or gender identity. If any participant has questions or concerns, please see #4 of this document and/or contact a Pathways’ staff member.
    
    
11. Names and addresses of Pathways’ participants as well as documentation of participation at Pathways are confidential. This information shall not be released without being authorized in writing by the participant.  There may be extraordinary situations in which Pathways will be required to release information about you pursuant to a court order; Pathways will make reasonable efforts to inform you about this type of a situation should it occur.
    
    
12. No routine records are kept on Pathways’ participants other than attendance records. Your Volunteer Provider may maintain other records about your participation at Pathways.  You have the right to access such records and written information in accordance with MN Statutes Sections 144.291-144.298.  You may consult with your Volunteer Provider for more information.
    
    
13. Other services are available by Pathways’ Volunteer Providers in their Private Practice and by other Complementary and Alternative Health Care Practitioners in their offices or clinics.  A list of those Pathway’s Volunteer Providers available for outside consultation is available on the Pathways Resources webpage.
    
    
14. Pathways’ participants have the right to choose freely from among the Volunteer Providers and may change Volunteer Providers pending their availability through the appointment procedures identified in each Pathways Calendar.
    
    
15. Each session provided at Pathways is considered an independent service. Therefore, there is no provision for coordinated transfer of on-going services to a different provider.
    
    
16. Pathways’ participants have a right to refuse Pathways’ services.
    
    
17. Pathways’ participants may assert the rights identified in this document without retaliation.
